What is PCMH?
The Patient-Centered Medical Home (PCMH) program is an innovative care delivery model that emphasizes evidence-based health care coordination and increased communication between patients and healthcare providers. The Agency for Healthcare Research and Quality describes the patient-centered medical home as encompassing five functions and attributes:
- Comprehensive care
- Patient-centered care
- Coordinated care
- Accessible services
- Ongoing commitment to quality and safety
Patient-centered medical homes can help facilitate more effective partnerships between physicians, patients, and caregivers by creating a central setting that is focused on giving patients the right type of care when and where they need it.
